Community Health Needs Assessment

Youth First & the Washington County Family YMCA are serving as community partners with St. Vincent Salem in their efforts to better serve the health care needs of our community.  St. Vincent Salem Hospital is currently conducting a Community Health Needs Assessment (CHNA). This is being conducted to identify the health needs of the community and meet new federal requirements that have been implemented with the passage of the Patient Protection and Affordable Care Act. All non-profit hospitals must conduct a community health needs assessment every three years in order to maintain their tax-exempt status under Section 501(c)(3).

Health information of community members is being collected through surveys that are being distributed by mail, and can also be found at:


Questions on this survey cover health topics ranging from smoking behaviors and exercise frequency, to types of health care coverage and health access difficulties that individuals may have encountered. The data gathered from these surveys will allow community leaders to prioritize the health needs of the community, ensure residents are connected to proper health services, increase access to care, and help improve the quality of life in the community. All information gathered through the surveys will remain confidential.

The deadline to complete the survey is March 8, 2013.

This survey is open to the entire community, so please forward it to your friends and family members for completion, as well. It is extremely important that we get at least 500 responses.
